A Note about Advance Directives

20th Street Surgery Center does elective procedures on an outpatient basis and, as such, does not honor particular elements of Advance Directives such as end-of-life decisions. In the event of a life threatening situation at this facility, advance cardiac life support will be instituted in every instance and the patient will be transferred to Saint Johns Health Center for further treatment.

If you want to formulate an Advance Directive we will provide you with information for you to review and California Advance Health Care Directive Forms. However, it is not recommended that you fill out an Advance Directive on the day of your procedure since some aspects of it will not be valid in the surgery center and it is a comprehensive legal document requiring careful consideration.

Patients will be informed of their rights to formulate and Advance Directive and that they are not required to have an Advance Directive in order to receive treatment at this facility. If you choose to prepare or provide an Advance Directive prior to your admission, we will retain a copy of it in your medical record and, in the event you are transferred to a higher level of care, the document will be sent with you.
